How do tesamorelin, sermorelin, ipamorelin, CJC-1295, and retatrutide differ?
Tesamorelin has clinical evidence for visceral abdominal fat in adults with HIV-associated lipodystrophy. Retatrutide has substantial obesity-trial weight-loss data. Sermorelin, ipamorelin, and CJC-1295 have different histories and much less directly applicable evidence for those same outcomes. A rise in growth hormone is not itself a measure of fat lost or muscle gained.
Evidence and regulatory status checked November 2, 2026. The studies below differ in population, formulation, duration, and endpoint; their percentages are not a head-to-head ranking. Schedules describe labeled treatment or specific research, not interchangeable personal protocols.
| Peptide | Pathway | Relevant evidence or goal | Frequency context |
|---|---|---|---|
| Tesamorelin | GHRH receptor; stimulates endogenous GH | Measured visceral-fat reduction in HIV-associated lipodystrophy.12 | Named EGRIFTA medicines use daily injections. |
| Sermorelin | GHRH(1–29) fragment | Historical pediatric GH-deficiency treatment and diagnostic use; small adult GH studies.3414 | Nightly injections in the older-men study discussed below. |
| Ipamorelin | Ghrelin-receptor GH secretagogue | Human infusion research measured GH response; it did not establish a fat-loss course.56 | 15-minute intravenous infusions in the cited study. |
| CJC-1295 with DAC | Long-acting GHRH analogue with albumin binding | Sustained GH and IGF-1 increases in healthy adults.78 | Single injections, or repeated every one or two weeks in trials. |
| “CJC-1295 no DAC” | Modified GHRH-based peptide without DAC | Distinct from the long-acting study material; exact identity matters.8 | The DAC trial schedule cannot be transferred to this form. |
| Retatrutide | GIP, GLP-1, and glucagon receptor agonist | Phase 3 obesity and diabetes research; investigational, not FDA-approved.911 | Once-weekly injections with escalation in clinical trials. |
Tesamorelin vs sermorelin: which has better evidence for belly fat?
Tesamorelin has the more directly relevant evidence for visceral fat in its studied HIV population. In two pooled phase 3 trials involving 806 participants, the placebo-adjusted reduction in visceral adipose tissue was 15.4% at 26 weeks. Abdominal subcutaneous fat did not change significantly.2 Visceral fat surrounds internal organs; this result is not a promise to remove the fat that can be pinched under the skin.
Sermorelin is the 29-amino-acid GHRH fragment, while tesamorelin is a modified 44-amino-acid analogue.114 Sharing the GHRH pathway does not make their outcomes interchangeable. An older study gave 2 mg GHRH(1–29) nightly for six weeks to 11 nonobese men aged 64–76 with low baseline IGF-1. GH release increased, but IGF-1 and DXA measures of muscle and fat did not change; some strength measures improved.4
Was sermorelin withdrawn because it was unsafe?
FDA withdrew the historical Geref approvals effective June 18, 2009, after the manufacturer requested withdrawal. In 2013, FDA determined that the products had not been withdrawn for safety or effectiveness reasons. Their uses were pediatric growth failure from GH deficiency and diagnostic testing.3 That history does not make a current compounded adult “anti-aging” product FDA-approved.
What is the difference between tesamorelin and ipamorelin?
They stimulate GH release through different receptor pathways. Tesamorelin acts through GHRH receptors; ipamorelin acts through ghrelin receptors. Neither is injected HGH itself.16
For someone comparing evidence for abdominal fat, the key distinction is the outcome measured. Tesamorelin's pivotal studies used abdominal imaging. The cited ipamorelin study gave healthy male volunteers escalating 15-minute intravenous infusions and measured peptide concentrations and GH release.5 It did not test a daily subcutaneous fat-loss or bodybuilding protocol.
Claims that ipamorelin is automatically a gentler substitute, or that adding it makes tesamorelin work better, need evidence beyond a receptor explanation. The tesamorelin–ipamorelin guide separates actual reported combinations from clinical findings and explains blend ratios.
Does CJC-1295 last longer than tesamorelin?
The studied long-acting CJC-1295 contains a drug affinity complex, or DAC. It binds to circulating albumin, extending exposure. In 28- and 49-day trials in healthy adults aged 21–61, the estimated half-life was 5.8–8.1 days. A single injection increased mean GH approximately 2–10-fold for at least six days and IGF-1 approximately 1.5–3-fold for 9–11 days.78
Those are hormone and pharmacokinetic results. They do not show that longer exposure produces more fat loss, better recovery, or more muscle than tesamorelin. The cited CJC trials did not establish those body-composition outcomes.
Is “CJC-1295 no DAC” the same peptide?
No. That name is used for a non-DAC modified GHRH(1–29) peptide, also marketed as Mod GRF(1–29). FDA distinguishes non-DAC and DAC forms and documents inconsistent naming. Its 2024 review identified the human clinical literature as involving DAC-containing material.8 The long half-life and infrequent dosing data above cannot be assigned to a vial merely because “CJC-1295” appears on its label.
Which is more relevant to overall weight loss?
Retatrutide's obesity trials directly measured substantial body-weight reduction; tesamorelin's approved use targets excess abdominal fat in HIV-associated lipodystrophy. Retatrutide activates GIP, GLP-1, and glucagon receptors, rather than being a GH secretagogue.9
| Evidence | Population and timepoint | Result |
|---|---|---|
| Tesamorelin pooled phase 3 trials | HIV-associated excess abdominal fat; 26 weeks | 15.4% placebo-adjusted reduction in visceral fat.2 |
| Retatrutide TRIUMPH-1 | Obesity or qualifying overweight without diabetes; 80 weeks | 12 mg weekly arm: 25.0% mean body-weight reduction versus 3.9% placebo, using the treatment-regimen analysis.910 |
The retatrutide analysis estimates outcomes regardless of treatment adherence or use of other weight-management interventions. Lilly also reported 28.3% for its efficacy analysis, which assumes continued study treatment without prohibited weight-management treatment. These are different analyses of the same trial, not conflicting measurements.10
Retatrutide remained investigational as of November 2, 2026. FDA states that it is not a component of an approved drug and cannot be used in compounding under federal law.11 A positive phase 3 result does not make an online research vial the medicine used in that trial.
Which evidence fits your goal?
Reducing visceral abdominal fat
Tesamorelin has the most directly relevant clinical program here for HIV-associated lipodystrophy. The result is narrower than general belly-fat advertising. For adults outside that population, the same response is not established by those trials. EGRIFTA is not indicated for weight-loss management.1
Losing a substantial amount of body weight
Retatrutide's evidence addresses that goal directly, but its investigational status matters. The relevant clinical discussion is obesity treatment and currently authorized options, not converting a study's target dose into a self-directed research-peptide cycle.
Building muscle or improving recovery
None of these sources establishes a universal bodybuilding winner. A tesamorelin analysis found favorable changes in muscle density and some muscle areas after 26 weeks, but it compared 193 selected visceral-fat responders with 148 placebo participants who had HIV.13 That is different from demonstrating strength or muscle gains in healthy lifters. An IGF-1 increase, improved scan measurement, and improved physical performance are distinct outcomes.
Can you swap equal milligram doses or use the same cycle?
No milligram-for-milligram conversion is established by these studies. Even tesamorelin formulations differ: EGRIFTA WR uses 1.28 mg daily, with exposure comparable to the original 2 mg formulation.1 That does not define an equivalent sermorelin, ipamorelin, or CJC-1295 amount.
The study durations also answer different questions: six weeks of GHRH(1–29), short CJC hormone studies, 26-week tesamorelin fat measurements, and 80-week retatrutide obesity outcomes are not interchangeable “cycles.” Use the dosage and reconstitution guide for named formulations, reported schedules, and concentration arithmetic.
Which peptide has fewer side effects?
The cited trials do not support a reliable safety ranking across all five compounds. Different participants, doses, follow-up periods, and adverse-event reporting prevent a fair comparison of percentages.
Tesamorelin's label identifies fluid retention, joint symptoms, glucose intolerance, elevated IGF-1, and hypersensitivity.1 Retatrutide's TRIUMPH-1 report includes gastrointestinal events and altered skin sensation; adverse-event discontinuation was 11.3% at 12 mg versus 4.9% with placebo.10
FDA separately flags limited clinical safety data and impurity-related immune concerns for ipamorelin acetate and CJC-1295. For CJC-1295, it also identifies reports of increased heart rate and systemic vasodilatory reactions.12 Fewer studies do not establish fewer risks. The tesamorelin side-effects guide covers its specific monitoring and contraindications.
Peptide comparison FAQ
Does a bigger GH or IGF-1 rise mean a better peptide?
No. A hormone response does not establish how much fat was lost, whether strength improved, or whether treatment was tolerable. Compare the outcome you want in a relevant population, alongside adverse effects.
Is CJC-1295 no DAC just another name for sermorelin?
Does combining two pathways guarantee better results?
No. Separate mechanisms provide a rationale to investigate a combination, not a measured benefit or a validated blend ratio. Read the tesamorelin and ipamorelin combination guide for the actual reported amounts, confounders, and evidence limits.
